Yes anyone hosting LAN or dedicated server needs it. Any HOST needs it, the clients connecting do not.

So, just to make things clear:  if I want to run a lan, do I also need to download the larger server .zip and then patch it with the hotfix, or do I just get the hotfix?

Download BOTH. Unzip the larger .zip first then the hotfix
