Does it refer to the map killing someone and the kill being accredited to someone?

Refers to you causing a prop in the world to kill someone else. ie: Anything explosive (gas canisters, generators, etc.), or managing to throw something at a high enough velocity to take someone out. (lockers, file cabinets, chairs, etc.)

If your killed with say a generator in Caves, its says "killed by World".

How do you get killed by a generator? I know you can break them to turn off the lights, but...

They explode.  The damage is pretty low but I've done it a couple times.  LTK makes it easy.
