I'm not sure what causes this bug, But I thought I should point it out, Since it happens quite often.

Would a teammate happen to be underneath that icon?

They are visible through walls up to a certain distance.

Not that I'm aware of, It layed down there for the whole round, Besides, It was right under Mayday, Not through the wall.
