Unfortunately I lost the original achievement I had made a few years back so I decided to remake it. I don't know if it will be accepted, but here's an image and a PSD if you want to take a look.

Phantastic Phantom
Criteria:
Get 100 kills with the Phantom [Agent]
Get 500 kills with the Phantom [Secret Agent]
Get 750 kills with the Phantom [00 Agent]
